The mf8 Crazy Pentahedron - Uranus is a 5 sided puzzle by mf8. This particular version has circular cutouts on all five of the faces with all of them being stationary relative to the side that they're on. In the graphic listed, sides labeled '1' have circular cutouts that rotate with the face, sides labeled '0' have cutouts that do not rotate with the face.
