Donor Management & Optimization — CPTC Study Guide | 2026 Edition

This guide is designed to help you master the hands-on clinical portion of the CPTC exam: Optimizing Organ Function. After authorization, the Organ Procurement Coordinator must recognize reversible donor abnormalities, select the correct intervention, and reassess organ function before a potentially transplantable organ is lost. CPTC Smart Study Guide: Optimizing Organ Function Optimizing Organ Function accounts for approximately 19% of the scored CPTC exam, or about 28 of the 150 scored questions on the current ABTC blueprint. This guide is built to the July 1, 2026 CPTC test specifications and pairs directly with the 50-question Optimizing Organ Function practice set. This guide helps candidates think like an Organ Procurement Coordinator: identifying the organ at risk, recognizing the reversible cause behind abnormal findings, choosing the intervention that directly treats the problem, and reassessing before prematurely declining an organ. Why This Guide Is Different Blueprint-Aligned: Built around the current ABTC Donor Management and Optimization domain, Task 2: optimizing donor organ function. Clinically Focused: Covers the critical-care interventions used to stabilize the donor and preserve the heart, lungs, liver, kidneys, and other transplantable organs. The “Exam Lens”: Teaches you to fix the cause—not simply react to the most abnormal number. You will learn to determine whether the best intervention is pharmacologic, procedural, or mechanical. The Reversible-Problem Mindset: Reinforces that low cardiac function, poor oxygenation, electrolyte abnormalities, infection, and other donor problems may improve with appropriate treatment and reassessment. What’s Inside 13 Numbered Study Sections: Organized around the clinical problems and interventions an OPC encounters during donor optimization. Hemodynamic Optimization: Covers perfusion, fluid strategy, vasoplegia, vasopressin, balanced crystalloids, diuresis, and lactic acidosis. Diabetes Insipidus and Metabolic Management: Reviews vasopressin versus DDAVP, hypernatremia, glucose control, and gradual correction of metabolic abnormalities. Electrolytes and Arrhythmia Prevention: Includes potassium, magnesium, phosphate, and calcium replacement—and when electrolyte correction should come before antiarrhythmic therapy. Pulmonary Optimization: Covers lung-protective ventilation, PEEP, FiO₂ titration, bronchoscopy, recruitment, prone positioning, bronchospasm, pneumothorax, and pulmonary edema. Cardiac Optimization: Explains why a low ejection fraction or elevated troponin may represent reversible myocardial stunning and should be reassessed after resuscitation. Hormonal Resuscitation: Reviews corticosteroids, vasopressin, refractory donor instability, and the current evidence surrounding thyroid-hormone use. Infection and Donor-Derived Risk: Teaches candidates to investigate, treat, document, and disclose infection or abnormal anatomical findings rather than automatically declining organs. Integrated Clinical Scenarios: Combines hemodynamic, endocrine, pulmonary, cardiac, and metabolic abnormalities into realistic CPTC-style cases. High-Yield Study Tools Interventions by Type: Pharmacologic, procedural, and mechanical interventions organized into a clear reference table. Reversible Problem → Intervention Workflow: Connects common donor abnormalities with the correct first action. Numbers and Targets to Memorize: Includes glucose, sodium, temperature, tidal volume, MAP, ventilation, electrolyte, fluid, and reassessment targets. Mnemonics and Exam Alerts: Includes “Pills, Procedures, Plug-Ins,” “Vasopressin vs. DDAVP,” “Return to Normal,” and “Optimize, Then Reassess.” Master Organ-Optimization Checklist: Connects perfusion, endocrine management, ventilation, electrolytes, cardiac reassessment, infection treatment, and risk disclosure. Includes Optimizing Organ Function CPTC Study Guide — Part 2 Hemodynamic and Fluid Optimization Diabetes Insipidus, Sodium, and Glucose Electrolytes and Arrhythmia Prevention Temperature and Acid-Base Management Pulmonary and Ventilator Optimization Cardiac Optimization and Reassessment Hormonal Resuscitation Infection and Antimicrobial Management Abnormal Findings and Donor-Derived Risk Integrated Donor-Management Scenarios Intervention Reference Tables Numbers, Mnemonics, Exam Alerts, and Checklists Don’t just memorize interventions—learn how to identify and correct the reversible problem. This digital study guide transforms complex donor physiology and critical-care management into a clear, student-friendly framework. It helps OPCs determine what is causing the abnormality, which intervention treats it, and when organ function should be reassessed. Exam Tip: Identify the organ at risk, name the reversible cause, treat the cause rather than the number, and reassess before declining a potentially salvageable organ. Every answer and rationale in this question set is source-checked against authoritative organ donation and transplant references, including HRSA/Organdonor.gov, the Revised Uniform Anatomical Gift Act, CDC/USPHS donor risk guidelines, American Academy of Neurology brain death guidance, and UNOS/OPTN policies.  Disclaimer This is not official ABTC content and does not reproduce ABTC questions. It is an independently written, evidence-based study product. For legal hierarchy questions, I used the Revised Uniform Anatomical Gift Act model hierarchy; actual OPO practice must still follow the applicable state law and OPO/hospital policy.
