You can leave it blank and we will look up your formula in our database based on your name. If you are purchasing it for someone else, just include their name in the Checkout notes and we will sleuth it out for you! *Your formula will be made exactly to the specifications of your original formula. You also have the option to upgrade your bottle size from a 30 ml to a 60 ml if you desire. If you would like any modifications made to it, please place your order online and then call the boutique at 570-343-2100 to discuss your changes with our Scent Designers.
